ABSTRACT

Paget's disease, described by Sir James Paget in 1874, is classified as mammary and extramammary. The mammary type is rare and often associated with intraductal cancer (93-100% of cases). It is more prevalent in postmenopausal women and it appears as an eczematoid, erythematous, moist or crusted lesion, with or without fine scaling, infiltration and inversion of the nipple. It must be distinguished from erosive adenomatosis of the nipple, cutaneous extension of breast carcinoma, psoriasis, atopic dermatitis, contact dermatitis, chronic eczema, lactiferous ducts ectasia, Bowen's disease, basal cell carcinoma, melanoma and intraductal papilloma. Diagnosis is histological and prognosis and treatment depend on the type of underlying breast cancer. Extramammary Paget's disease is considered an adenocarcinoma originating from the skin or skin appendages in areas with apocrine glands. The primary location is the vulvar area, followed by the perianal region, scrotum, penis and axillae. It starts as an erythematous plaque of indolent growth, with well-defined edges, fine scaling, excoriations, exulcerations and lichenification. In most cases it is not associated with cancer, although there are publications linking it to tumors of the vulva, vagina, cervix and corpus uteri, bladder, ovary, gallbladder, liver, breast, colon and rectum. Differential diagnoses are candidiasis, psoriasis and chronic lichen simplex. Histopathology confirms the diagnosis. Before treatment begins, associated malignancies should be investigated. Surgical excision and micrographic surgery are the best treatment options, although recurrences are frequent.

ABSTRACT

Mammary Paget's disease is a rare intraepithelial adenocarcinoma, located on the nipple/areola complex, highly associated with breast cancer. Although the international literature emphasizes the dermatoscopic pattern of mammary Paget's disease pigmented variant, the authors describe the dermoscopic findings of classical Paget's disease and demonstrate the presence of chrysalis-like structures, criteria recently described in the literature and not yet reported in Paget's disease. .


Doença de Paget mamária é considerada um adenocarcinoma intra-epitelial raro, localizado no complexo mamilo-aréola,com alta associação ao câncer de mama. Apesar da literatura mundial realçar o padrão dermatoscópico da doença de Paget mamária variante pigmentada os autores descrevem os achados dermatoscópicos da doença de Paget clássica realçando a presença das estruturas crisálida-símiles,critério recentemente descrito na literatura mundial e ainda não relatado na Doença de Paget. .

ABSTRACT

Reportar y describir las características clínicas y conducta terapéutica de 13 casos de enfermedad de Paget en el Instituto de Oncología “Dr. Miguel Pérez Carreño” en el período comprendido entre 1986-2008. Se realizó un estudio no experimental, descriptivo, de tipo retrospectivo. Se revisaron historias clínicas del departamento de dicha institución. Total de casos encontrados: 13, 100 por ciento de los pacientes fueron del sexo femenino. La edad promedio de presentación fue de 58 años. 44 por ciento de los casos se presentó con una lesión eccematosa a nivel de pezón y un 14 por ciento se encontraban en estadio I en el momento de diagnóstico. 87 por ciento presentaban extensión centro-mamaria; el tiempo de evolución fue menor o igual a 6 meses en un 50 por ciento de los casos. El síntoma de presentación en un 45 por ciento fue la presencia de un nódulo mamario más lesión ulcerativa del pezón. 25 por ciento cursaba con adenopatía clínicamente palpable. En un 65 por ciento, la conducta quirúrgica fue mastectomía total con vaciamiento axilar en niveles I y II. 37 por ciento recibieron tratamiento adyuvante y en un 72 por ciento el tiempo de sobrevida comprendió entre el rango de 1 a 5 años. La enfermedad de Paget constituye una entidad especial poco frecuente. El grupo etario afectado está entre 50-60 años. El tratamiento terapéutico establecido en la mayor parte de las series publicadas, independientemente de la etapa, ha sido la mastectomía radical.


The objective of this study was report and describes the clinical characteristics and therapeutically conducts in 13 cases of Paget disease view in the Oncology Institute Dr. Miguel Perez Carreño between the years 1986-2008. We realized a study not experimental, descriptive and retrospective. We review the clinical department history of our institution. The total cases found was 13, 100 percent of the patients were female. The average age at the presentation was 58 years old. The 44 percent of the cases were presented with an eczematous lesion in the nipple level, 14 percent of the cases were in stage I at the diagnostic moment. The 87 percent of cases present a center mammalian extension to the diagnostic moment. The evolution time were minor or equal to 6 month in a 50 percent of the cases. The dominant symptom in 45 percent was the presence of a mammalian nodule and an ulcerative lesion in the nipple. In the 66 percent of the cases the surgical conduct was a total mastectomy with axillaries dissection in the level I and II. Only 37 percent receive adjuvant treatment and in a 72 percent the super life were between the 1 and 5 years range. Paget disease is a less special entity. The average groups affected are between the 50 and 60 years old. The established therapeutically treatment in the majority of series published independent of the stage was the surgical.

ABSTRACT

La enfermedad de Paget mamaria es una patología poco común en la mama, sin embargo, tiene gran importancia por su alta asociación a carcinoma mamario y porque requiere de un alto índice de sospecha. Se presenta el caso de una mujer de 56 años con enfermedad de Paget mamaria con focos microscópicos de invasión y compromiso linfonodal masivo. Se realiza una revisión de esta patología, con especial énfasis en la sospecha, diagnóstico, manejo, estadificación, tratamiento y pronóstico desde el punto de vista médico y quirúrgico.

ABSTRACT

Antecedentes: El carcinoma de paget es una variedad de cáncer de mama caracterizado frecuentemente por la asociación temprana de una lesión de piel de areola y/o pezón que proviene de los conductos galactóforos. fue descripto en 1874 por Sir James Paget. Objetivo: Analizar los casos presentados y hacer una breve historia de la enfermedad, histogénesis, biología, fisiopatología, diagnóstico, diagnóstico diferencial y tratamiento. Lugar de aplicación: Hospital Público y de Obra Social. Diseño: Estudio retrospectivo. Población: 5 casos, todas mujeres, en 528 cánceres de mama (0,95%). La edad media fue de 63 años; 4 fueron invasores y 1 "in situ", 3 de ellos tenías tumor palpable. El diagnóstico fue hecho por biopsia diferida de pezón, galactóforos terminales o areola. Método: El tratamiento fue quirúrgico en todos los casos; en 4 se realizó mastectomía radical modificada y en 1 cirugía conservadora con resección del complejo areola-pezón. Resultados: La supervisvencia global fue de 60% con 19 años o más de seguimiento; sobreviven libres de enfermedad 1 caso con tumor palpable, y 2 con tumor no palpable, 1 infiltrante y 1 "in situ"; fallecieron 2 de los 3 casos con tumor palpable. Conclusiones: El carcinoma de Paget es un carcinoma ductal epidermotrofo de la mama, a veces multifocal. El único tratamiento con intención curativa es el quirúrgico, ya sea por mastectomía radical modificada, lo habitual, o cirugía conservadora más redioterapia para los casos iniciales no infiltrantes o sin tumor palpable. ABSTRACT

Mammary Paget's disease is one clinical expression of an underlying breast carcinoma. It manifests as persistent erythema, oozing and scaling of the nipple, areola or both. The occurrence of this disease in men is uncommon. This is a report of a pigmented lesion of Paget's disease occurring on the breast of a man. This presentation is unusual and may be confused with melanoma. Its early recognition is important so that adequate therapy may be instituted when the disease has a favorable prognosis

ABSTRACT

Many breast carcinomas probably arise in a multi-step fashion through a series of intermediate lesions viz. ductal hyperplasia to atypical ductal hyperplasias to ductal carcinoma in situ (DCIS), and thence to invasive ductal cancer, each of which has a greater probability of becoming malignant than the one that preceded it. These precursor lesions have differing risk implications, hence treatment decisions vary with the risk. The heterogeneous lesions that come under the heading of hyperplasias and DCIS can cause problems for the histopathologists unless there is a conceptual understanding of the disease process. Since the surgical pathology report is the final word in cases of DCIS/LCIS, the pathologist becomes a vital partner in the decision making team.

Os autores apresentam um caso de doença de Paget da mama em homem, com documentação do estudo anatomopatológico e imunoistoquímico, da mamografia, do linfonodo sentinela e do tratamento preconizado. Discutem ainda a importância do estudo desta patologia no homem e as questões relativas ao prognóstico.


The authors present a case of Paget's disease of the male breast showing pathological and imunohistochemical studies, mammography, sentinel lymph nodo evaluation and the given treatment. They also discuss the study of the pathology in man and the prognosis.

ABSTRACT

La enfermedad de Paget mamaria es una neoplasia de carácter maligno, desarrollada principalmente en la areola mamaria y pezón, que se diagnostica por sus características histológicas específicas. Se considera una manifestación cutánea de un carcinoma subyacente, ya que en casi todos los casos se detecta una neoplasia maligna mamaria. La enfermedad de Paget extramamaria es una neoplasia intraepidérmica que se ubica en zonas donde existen glándulas apocrinas. Esta entidad se diferencia de la enfermedad de Paget mamaria en cuanto a su histogénesis, características tintoriales y frecuencia relativa de carcinomas subyacentes. En este artículo se describen las características más importantes de ambas entidades.


Paget's disease of the breast, diagnosed by specific histologic characteristics, is a malignant neoplasm that develops mainly in the areola and nipple. It is considered a cutaneous manifestation of an underlying carcinoma because a malignant neoplasm is almost always detected. Paget's extramammary disease is an intraepidermal neoplasm that is located in apocrine gland regions. This entity differs from Paget's mammary disease with respect to its histogenesis, staining qualities and relative frequency of underlying carcinomas. In this article we describe the most important characteristics of the both entities.

ABSTRACT

Technetium-99m methoxyisobutylisonitrile (99mTc-MIBI) uptake is known to be increased in breast cancer because of increased blood flow from angiogenesis and heightened metabolism. We performed a 99mTc-MIBI scan in a patient with mammary Paget's disease. The patient had underlying invasive cancer in the same side of the breast. 99mTc-MIBI scan exhibited a scintigraphic image of the uptake from the invasive cancer lesion located deeply in the breast toward the epidermis. 99mTc-MIBI showed an uptake in the deeply located invasive cancer lesion as well as nipple lesion. Especially, the delayed phase of Tc-MIBI scan demonstrated the tumor site more accurately. In conclusion, 99mTc-MIBI scan could be a useful adjunct to clinical decision making in the management of Paget's disease of the breast.

ABSTRACT

La enfermedad de Paget extramamaria anaplásica y acantolítica (EPeMAA) y la enfermedad de paget mamaria anaplásica y acantolítica (EPMAA), son formas raras de la EP; no se sabe en el momento actual su frecuencia real. Se presentaron por arriba de la cuarta decada, en 4 mujeres y 3 hombres. Su evolución fué crónica, clínicamente fueron placas verrugosas, ulceradas, facilmente sangrantes, bien limitadas. Clínica e histológicamente se confundieron con enfermedad de Bowen y con el pénfigo vegetante. Microscópicamente se observó en todo el espesor de la epidermis células anaplásicas pequeñas, con núcleo ovoide, cromatina fina sin disqueratosis, escaso citoplasma; la acantólisis fué acentuada con formaciones vegetantes. Se observaron células clásicas de Peget en forma alterna; todos exhibieron ulceración y en la periferia la imagen típica de EP. En la base de la neoplásia hubo infiltrado linfocitario en banda "patrón liquenoide". En 4 de los casos se identificó neoplásia maligna sincrónica en la profundidad (1 caso Ca de conductos mamarios, 2 casos Ca de conductos de glándulas apócrina y 1 caso de Ca. de conductos de glándulas ecrina de tipo esclerosante) fueron positivos al antígeno carcinoembrionario (ACE) 5/5, al antígeno de membrana epitelial (AMA) 5/5, y a la citoqueratina de bajo peso molecular (Cqbpm) 5/5. Hay que hacer el diagnóstico diferencial con la enfermedad de Bowen (EB), melanoma de diseminación superficial (MMDS) y con el pénfigo vegete. No hay diferencias en pronóstico entre la EP mamaria y extramamaria clásica y la EPeMAA u EPMAA

ABSTRACT

Paget's Disease of the breast is caused by spread of duct carcinoma cells along the mammary ducts to the epidermis of the nipple and areola. This is a study of 43 cases of Paget's Disease of the breast. Though only few patients presented with a lump; a carcinoma, either DCIS or IDC or both were found in all cases. The presence of an underlying breast carcinoma in Paget's Disease of the breast suggests that radical mastectomy is the treatment of choice in this condition.
